Medication History for Populations helps health systems, hospitals, accountable care organizations and health plans manage cost-effective care for patient populations by providing access to medication history and proactive prescription notifications. With this new service, Particle Health customers will have access to up-to-date medication lists for patients, as well as insights to understand whether patients are filling their prescriptions on time and as directed, allowing providers to identify early signs of non-adherence and intervene promptly to prevent complications like unnecessary hospitalizations.

This prescription history data from nearly all major pharmacies and pharmacy benefit managers helps clinicians and care providers pinpoint those at risk of nonadherence, adverse drug events (ADEs) or potential abuse—and easily triage patients and members by risk group to support quality improvements.
